Torfaen Community Connectors
Community Connectors work with individuals, groups and organisations across Torfaen.
Their main objective is to support people who may feel isolated, lacking confidence or who have lost their independence. They will listen to what individuals want and need at the point of making contact.
During any contact/visit or time spent with the person the Community Connectors will support someone to be more confident, improve their health and well-being. They will find out what’s going on in the person’s area and how they can get involved in groups, community activities or courses, while trying to reduce isolation and support people to be more independent.
They will work with someone to focus on the individual’s strengths and capabilities. Community Connectors also start up groups within the community if there is a need or interest from several people. They currently facilitate two friendship groups, a games group, confidence building group and art group. They will support people to set achievable goals for a happy and enjoyable lifestyle. Community Connectors do this by connecting people to other people, information, local services and tailor support accordingly to maintain individuals independence and reduce isolation.
